Skilled Nursing and Rehabilitation at Brooke
Grove Retirement Village
The Brooke Grove Rehabilitation and Nursing Center is a state-of-the-art
facility built in 2000. It is home to residents who require intensive
nursing, custodial, short-term rehabilitative or hospice care.
The nursing center is Medicare- and Medicaid-certified.
Services Include:
• Long-term, Short-term and respite care (Link to Respite

• 24-hour skilled nursing care
• Physical, occupational and speech therapies
• Assistance with the activities of daily living, including
bathing and dressing
• Administration of medications, including IV medications
• Weekly outings, weather permitting
• Daily activities program
• Personal laundry and linens
• Housekeeping
• 24-hour skilled nursing care for individuals requiring
short- or long-term nursing or rehabilitation services
• Occupational, physical and speech therapies
• Dining program supervised by a registered dietician and
nutritionist, with meals prepared based on individual preferences
and nutritional requirements
• Linen, laundry and housekeeping services
• Customized service plans
• Daily social, therapeutic and recreational activities
• Holiday parties, intergenerational activities, pet visits
and excursions
• Chaplaincy services and spiritual activities for all faiths
• Full-time licensed social worker
• Family Council meetings
• Specialized Alzheimer’s care
• Priority access to Brooke Grove’s assisted living
services
• Respite care to offer family members a break or a chance
to check out Brooke Grove on a trial basis; minimum stay of seven
days

• 100% fresh-air intake ventilation system for heating and
cooling

Respite Care
For those times that you may need a break from caring for your
loved one, Brooke Grove is here to help. Respite care on our campus
can be from 3 days to several weeks.
